I solved my issue by installing a newer driver than 17.1.1, 17.7.2.
CRU is now working.

UPDATE--
So, I've wasted about 6 hours of my time installing and reinstalling drivers and I found out that 17.2.2 is the best driver for the Lenovo IdeaPad G40-45, probably also for other laptops with a Mobile A series in it.
Sure, you can install the latest driver but you won't have the ability to assign a certain app to use a specific power plan (just like in the AMD Additional Settings / Catalyst Control Center).
Not to mention CRU also works perfectly with 17.7.2...

I've noticed that since I installed Windows 10 2004 some months ago, it "looses" CRU custom settings much more easily than before when they lasted 'till next NVidia driver update... Now my CRU custom res sometimes randomly just disappear and I have to relaunch CRU + restart64.exe. Someone knows why and if there's a "solution" to make custom settings more persistent?

Basically every day I have to reload CRU + Restart64.exe to make my custom res reappear. There was a time when I just set it once and lasted 'till the day I changed video driver. Now for unknown reasons after a night sleep+hibernation the system looses my custom res. If I do sleep or hibernation manually for testing this does not happen and CRU custom res are still there....

EDIT:
Another strange thing I noticed is that I have 2 monitors listed in CRU and in the registry (??), that differ just for "Address" and "ContainerID" values:

EDIT2: from Device Manager (show hidden devices option) I deleted the strange one "1&8713bca&0&UID0". Let's see if this helps...

...and once again Windows re-created the ghost monitor "1&8713bca&0&UID0" (!?)
I've also opened a thread on NVidia forum, as I'm quite sure at this point it's something broken with recent drivers (and/or Windows 10):

That's not normal, and I can't think of what would cause that. I don't think it's an NVIDIA driver bug because it wouldn't delete the EDID override. Even installing a new driver shouldn't delete the EDID override unless you're using DDU to wipe everything first.

Maybe the computer is crashing while hibernating so the settings are reverting to the last booted configuration. Have you tried rebooting instead of running restart64.exe?

The "ghost" monitor is not a problem. That gets created when restart64.exe restarts the graphics driver.
No there's no crash or anything strange overnight... EDID override key in the registry is still there even when this happens. To me it looks a bug in Win 10 2004 or NVidia...
When I say "res disappeared" I mean won't available anymore for the OS, and in NVidia driver panel:

...normal behaviour with 60, 90, 120Hz res created with CRU still listed and working.
When the problem occurs instead they disappear and NVidiaCP will show just native 144Hz res, even though the registry will still have the EDID override key intact Huh
Opening CRU will show me the list of my custom res as expected (they've not been deleted), and all I have to do in this case is to use your Reset64.exe and everything will work again. It's like they get just ignored for some reason...
I'm not alone, at least another user reported the same problem with CRU + NVidia 452.06+

Rolled back to 451.77 SD (WHQL) and I can confirm that tonight with usual Sleep+Hibernation it DID NOT HAPPEN: all custom screen res created with CRU on resume were still active and working.

EDIT: Damn it! It happened again even with 451.77 Sad Could it be a Windows 10 2004 bug at this point...?

Wait, if it's listed in the Windows display settings but not the NVIDIA control panel then it's definitely an NVIDIA driver bug. I've seen a similar NVIDIA driver bug under different circumstances. The bug triggers when there's a hardware scan. See if you can get the bug to trigger after restarting the graphics driver by using Device Manager (right-click on anything and choose "Scan for hardware changes").

To work around the bug, try adding an CEA-861 extension block. You don't have to put anything in it. If that doesn't work, do you have GeForce Experience installed? Try uninstalling it. I remember GeForce Experience would trigger a hardware scan when it starts. Samsung Magician is another program that triggers a hardware scan.

Tried as you suggested to trigger the bug but nothing wrong happens rescanning for hw changes and screen res are still listed under NCP.
Yes I use GeForce Experience... next thing I'll try is to remove it.

Found how to trigger the bug: just closing laptop cover (enables sleep mode).
Manual Sleep or Hibernation do work fine, but when I use the laptop lid it triggers the bug on resume re-opening the laptop!
So at this point I tried without GeForce Experience, and was still reproducible Sad

Must be an NVidia driver bug or Windows 10 2004 (19041.508), at this point.
Rolled back to 451.77 DS but still happens...

EDIT: still reproducible even with 451.48...
