Add HDMI support, not HDMI 2.0, and you need to have at least one detailed resolution for the native resolution. Are you sure the GPU is dedicated and not switchable? If it's dedicated, CRU should have been able to read the extension block. If it's switchable, Intel's GPU takes over the inputs, and I don't know if Intel supports 4:2:0 resolutions.

The computer is Lenovo y50-70 - intel graphics 4600hd (i7 4720HQ) with Geforce 960m
I did a reset to the CRU application and tried again like you said
Here in the picture you can see I added HDMI support and 4:2:0 resolution with the resolution I want 4K60HZ.
