Another issue is the video driver will add some lower scaled resolutions regardless, and those resolutions will use the refresh rates of the higher resolutions. The only way I know to get around that is to not have those refresh rates at all for any resolution. I don't know of a way to make a certain refresh rate the default for a resolution.
