Are you sure the driver is patched? Run the patcher again to make sure, and make sure you are using the latest version.

I don't see anything wrong with the settings you posted, except I would have 1920x1080 @ 60 Hz as a detailed resolution instead of a standard resolution. Try removing all resolutions except 1920x1080 @ 120 Hz with the VT tweak and 1920x1080 @ 60 Hz as detailed resolutions. If that doesn't work, maybe there's a new driver limit that prohibits large vertical blanking intervals.
