I only used NVIDIA Control Panel to get the numbers for CRU, then i never used custom resolutions through NVIDIA Control Panel. For all tests after i found this issue i turned off DSR (so issue present with or without it). About 60Hz: when i have default timing on 60 and custom on 87 i can get rid of artifacts by witching to 60 and back to 87; if i have custom timing on both 60 and 87 this trick doesn't work. By strange resolution i meant one with not 16:9 ratio (cause i only had option to test it with XIII and dosbox, but then i find out that in gta 5 i can turn on something line 1680x1050 at 60 and issue gonna be triggered again). Screenshot shows the setting i settled with after trying everything you said... well i guess everything except i forget to set 87Hz as my first detailed resolution (or i set it when tv resolutions was on), because now i tried setting 87 as firs detailed and deleting everything except 60 and it works.
