It doesn't look like a full reset with just the detailed resolutions. It looks like you imported an old hdmi-audio.dat file, which replaces the extension block. How did you do the reset?

For 3840x2160 @ 60 Hz, you need to use the "LCD native" timing parameters. It looks like you used the "LCD reduced" option, which probably won't work with a TV. The TV should have already defined this resolution by default, although maybe it was in the extension block. For the other detailed resolutions, all the LCD timing options are the same, so they shouldn't be affected. 3200x1800 should work unless the TV doesn't support it.
