The HD 4200 is too old for the patch. The patch is for 5000-series and newer GPUs. HDMI is limited to 165 MHz pixel clock on 4000-series and older GPUs. You can't run a 1440p monitor on HDMI with 4000-series GPUs except at lower refresh rates like 40 Hz, if the monitor accepts that.
